New Patient Information
Are you accepting new patients?
Yes! We’re here to assist you with expert care and guidance to help you reach your optimum health and feel better. Please request an appointment online or contact us at +1 (904)-724-5767 to get started today.
What do I need to become a patient?
Before making your first appointment, our on-site physician will need to review any prior medical records and diagnostic study reports such as MRI, CT or X-rays. You will need to provide these to us electronically (email or fax) or drop them off at our office in-person.
If you need assistance with getting imaging studies done, please call us and ask about our referral programs, which can save you money.
How can I send you my records?
You can fax, email, or drop them off to our office.
How long does it take to schedule an appointment after sending records?
After the doctor has reviewed and approved your records, we can schedule your first appointment as soon as the next business day.
Where are you located?

Special Offers, Insurance & Billing
Do you accept insurance?
We are a self-pay practice. If you have any commercial insurance, BCBS, Aetna, Cigna, UHC, etc.. we will file your claims for you as a courtesy. If you have any out of network benefits, then your insurance should send any checks directly to you.
Can I fill my prescription with Medicaid?
Dr. Beaman is not a Medicaid provider, so you cannot use your Medicaid to cover any expenses associated with your visit, ie., Prescriptions, MRI’s, X-rays, or office visits. If you have any questions, please contact our front office at 904-724-5767.
What if i am not satisfied with the care i receive?
The quality of your care is our highest priority, and we’re dedicated to giving you the best experience possible. If we ever fail to meet your expectations, we’ll do our best to fix it.
Can I pay with a debit/credit card?
Yes, we accept Visa, Mastercard, Discover, American Express. We do offer a discount for paying with cash.

Prescriptions & Virtual Appointments
Where can I fill my prescription?
You may fill your prescription at any pharmacy that will accept it. For more information, please contact our front desk at 904-724-5767.
Can I schedule a tele-health visit or virtual appointment?
Currently, the doctor is required to physically see you whenever a prescription for a controlled substance is prescribed.
